Benefits Book & SPDs

The Benefits Book, Effective January 1, 2016, is your guide to many benefits for 2016. It contains Summary Plan Descriptions (SPDs), which describe the provisions of certain Wells Fargo-sponsored benefit plan options for benefits-eligible team members on U.S. payroll. Please refer to the Benefits Book by clicking the link below to learn, such as what's covered, how to file claims, or how to change coverage.

SPDs for the medical plan options vary depending on the specific plan selected. SPDs for the self-insured medical plan options are included in the Benefits Book. To obtain the portion of the SPD describing medical benefits available for an insured Kaiser medical plan option, please contact Kaiser directly.

The Benefits Book is revised annually and any changes for the new plan year will be described in the updated Benefits Book, generally available in January of each year.

Medicaid and the Children's Health Insurance Program

The Children's Health Insurance Program (CHIP) provides health coverage to eligible children, through both Medicaid and separate CHIP programs. CHIP is administered by states, according to federal requirements. The program is funded jointly by states and the federal government.
