How the Consumer Directed Health Plan Features Work
The UnitedHealthcare Consumer Directed Health (CDH) Plan features two plans. The CDH Gold and Silver Plans deliver two levels of coverage with respective deductibles, coinsurance, and premiums. Both plans include these key components:
Health Reimbursement Account (HRA). At the beginning of your benefit year, Wells Fargo contributes dollars to your HRA:

Your HRA pays first dollar coverage for eligible medical and pharmacy expenses. See the Rates and Comparison Charts 2009 for details. Also, your HRA rolls over. If you do not use all of your HRA — and remain enrolled in the UnitedHealthcare Consumer Directed Health (CDH) Plan — your remaining balance will roll over into the following year's HRA as long as you remain in the plan.
Deductible. If you use all of the funds in your HRA, you then pay for additional health care expenses up to the amount of your plan’s deductible. Remember, your share of the deductible will be less if you carry over HRA dollars from the previous year.

Coinsurance. You may have eligible health care expenses that exceed your deductible. If this happens, coinsurance takes effect to cover a portion of your eligible expenses. In these instances, if you use a network provider, you pay 20% of medical expenses (10% for maternity) after the HRA and deductible are met.

Out-of-pocket maximum. To protect you, an annual limit applies to the amount you are required to pay out of pocket. This limit is called your out-of-pocket maximum. If you reach this maximum, your in-network eligible expenses are covered 100% by the plan for the remainder of the plan year. See the Rates and Comparison Charts 2009 for details.
Preventive care. Most preventive services — such as physical exams, immunizations, and cancer screenings — are covered at 100% when you use a provider within the UnitedHealthcare network (see the Provider Directory Service). There’s no cost to you — based on the annual exam schedule — and you may use your preventive care benefit at any time, without having to first meet your deductible.
Prescription Coverage
Medco manages the prescription drug benefits for the CDH Plan. Participants receive benefits through Medco's network of pharmacies as well as mail-order service for medications you take long term.
Retail prescriptions. When filling prescriptions at a retail pharmacy, always present your member ID card. It contains all the necessary information for the pharmacy to process your prescriptions.
Mail-order prescriptions. Use mail order for most medications you take on an ongoing basis. Ask your doctor to write a prescription for up to a 90-day supply, plus refills for up to one year, as appropriate. To fill your prescription, do one of the following:
