The HSA High Deductible Health Plan (HSA-HDHP) features a high deductible health plan plus a health savings account (HSA) partially funded by Wells Fargo and administered by Wells Fargo Health Benefit Services.
How the Plan Works
Coverage includes access to the UnitedHealthcare Choice Plus provider network (Medica Network in MN, ND, SD).
Preventive care is covered 100% if you use a network provider.
All other care including, nonpreventive medical care, mental health and substance abuse care, and prescription drugs are subject to the plan's deductible and coinsurance provisions.
In-network deductibles of $2,000 for individual and $4,000 for family must be satisfied before the plan pays 80% of eligible claims.
The maximum annual out-of-pocket for in-network expenses is $4,000 for individual and $8,000 for family, including the deductible.
Wells Fargo contributes $350 for individual coverage, $750 for family coverage to an HSA each year.
You may contribute additional tax-free funds to your HSA.
How Funds Are Used
HSA funds may be used to pay for deductible and eligible medical expenses, or you may save your funds HSA funds for future use.
HSA funds left in the account each year roll over to the following year for future health care expenses.
When you leave Wells Fargo, you may take any unused funds in your HSA with you.
