The UnitedHealthcare Consumer Directed (CDH) Plan (formerly the Wells Fargo Definity Health Plans) provides a unique alternative to HMOs and other traditional health care plans. The CDH Gold and Silver Plans deliver two levels of coverage with respective deductibles and coinsurance. See the Rates and Comparison Charts 2009 for details.
Available in all states except Hawaii, the CDH Plan includes:
Preventive care with a network provider covered 100%.
Eligible medical and pharmacy expenses paid first with Health Reimbursement Account (HRA) dollars.
Ability to roll over unused HRA balance from year to year, if you remain enrolled in the UnitedHealthcare CDH Plan.
The CDH Plan is similar to preferred provider organization (PPO) plans in that there is a preferred network of physicians and hospitals. You don’t need to designate a primary care physician and you can see a specialist at any time. But your benefit coverage will be greater when you use a provider within the UnitedHealthcare network (see the Provider Directory Service).

Prescription Coverage
Medco manages the prescription drug benefits for the CDH Plan. Participants receive benefits through Medco’s network of pharmacies as well as mail-order service for medications you take long term.
Retail prescriptions. When filling prescriptions at a retail pharmacy, always present your member ID card. It contains all the necessary information for the pharmacy to process your prescriptions.
Mail-order prescriptions. Use mail order for most medications you take on an ongoing basis. Ask your doctor to write a prescription for up to a 90-day supply, plus refills for up to one year, as appropriate. To fill your prescription, do one of the following:
Mail your prescription(s) along with the “Medco By Mail Order Form” found on www.medco.com/openenroll (password: wellsfcdhpfsa). You can also get a copy of the form by calling the number on the back of your ID card. Call 1-888-327-9791 for instructions on how to fax the prescription.
