Changes for All Team Members

  • Rate changes. Most medical plan options, dental plan options, and the Long-Term Disability Plan have rate increases, so participants’ per-paycheck contributions will increase next year.
  • Benefit changes. Some medical plan options have benefit changes. Review this guide to make sure you know what’s changing and to ensure that your elections still fit your needs.
  • Certify Your College-Age Dependents. New requirement to certify your college-age student dependents during Annual Benefits Enrollment.

Plan Design Changes —
UnitedHealthcare PPO Plan (Formerly Wells Fargo Health Plan)

  • Coinsurance. Coinsurance is increasing to 20% from 10% for in-network hospital and outpatient services and to 40% from 30% for out-of-network services.
  • Indemnity. Participants who do not live within the Choice Plus Network (formerly Wells Fargo Health Plan Indemnity/PPO) will no longer have a coinsurance differential and will pay a coinsurance of 20% after satisfying the deductible.
  • Emergency Room. In-network emergency room benefit will change from $100 copay in network to 20% coinsurance.
  • Prescriptions. The prescription drug benefit is changing to a low copayment for generic drugs and a higher coinsurance for brand-name drugs. The plan changes are designed to encourage you to use generic drugs and switch to mail-order services for drugs you take long term.

Note: Beginning next year, participants who use mail-order services will have the financial protection of an annual out-of-pocket maximum for mail-order prescription drug purchases. See the chart below.

Other Changes

Medical Plans

Keep in mind that medical plans will only be available based on your home ZIP code. Previously, some plans were available based on both work ZIP code and home ZIP code. Please make sure your home address is correct to ensure you are offered the right plans.

Wells Fargo Dental Plan

Delta Dental is changing bitewing x-ray coverage for adults from once every 24 months to once every 12 months. Learn more about the dental plans.

Flexible Spending Accounts

Participants in the Health Care Flexible Spending Account and/or Day Care Flexible Spending Account will have more time to file claims.

Day Care Flexible Spending Account Grace Period

Participants in 2009 will have until March 15, 2010 to incur eligible expenses, similar to the grace period allowed for the Health Care Flexible Spending Account. The extended claims grace period allows you to have more time to use your dollars. For details, see the Day Care Flexible Spending Account section.

Dependent Certification College-Age Student Dependents

Wells Fargo has made it easier for you to certify your college-age student dependents. Prior to this year’s annual enrollment, if you had college-age student dependents you were required to certify that they were full-time students directly with your medical or dental plan. Instead, Wells Fargo now requires you to certify online once a year.

Newly Enrolled Dependents

If you add new dependents to your benefit plans, you will also be required to certify their eligibility online. To certify, see the online dependent certification instructions
