Enroll online from home or work. Review your options and enroll online if you want to make changes or need to make new elections because of plan eliminations or plan additions (see Special notes below). In addition, you must reenroll if you would like flexible spending accounts for 2010 — even if you are enrolled for 2009 — because enrollment does not carry over.
The UnitedHealthcare PPO Plan for 2010 will no longer have copayments. Instead, for all services, including office visits, you’ll be required to satisfy a deductible first, then you’ll pay 20% of the cost. Annual physicals and well-child exams are covered at 100%.
Basic and Optional Long-Term Disability plan benefits have changed and the enrollment process is different. Liberty Mutual is the new carrier.
For Optional Term Life Insurance, a new group term life insurance plan will replace your Group Universal Life Plan. You may also increase your coverage one level without proof of good health.
